Maternity Benefits Are Now Available To HealthAmericaOne Members 

Effective June 1st, 2012 policy effective dates for all Choice1 and Copay Series plans.

Benefits include:

• A $2,500 Inpatient copay for hospital services. The deductible and coinsurance do not apply
• Specialist OV Copay for first prenatal visit only.

1. o$20 Copay for Choice1 series plans
2. o$40/$50 copay for Copay series plans

• Deductible and coinsurance apply to any other maternity service. For example, physician charges for delivery and diagnostic tests. 
• All maternity services are subject to a nine-month waiting period. 

*Maternity benefits apply to new business effective dates of June 1, 2012 and later. Existing subscribers seeking maternity coverage must re-apply for the plans and will be subject to underwriting approval for enrollment. Maternity benefits available with Choice1 and Copay series plans only.

Other Changes Effective 6.1.12:

1. We have implemented a new application for the addition of the maternity benefit on the Copay and Choice1 series.  Click here for the most recent version of our application.  The application can also be found in the Broker Tool Kit under the "Enrollment" section.  Please begin using this application immediately for all of your new business submissions 6.1.12 and later effective dates.

2. We have retired the Copay 90% $750 and Copay 90% $1000 plans effective 5.31.12 for new business submissions.  Members currently enrolled may stay on this plan at their renewal.
